Portiko Alpha · 2026

Operational tools
for UK healthcare teams.

Cadence is a clinical-governance operating system for UK GP practices and care-home governance teams, currently in closed alpha. Relay, Quorum, Beacon, and Atlas extend the library into daily workflow, committee continuity, assurance posture, and cross-system integrity. GovernReady and the CESR Evidence System are two working kits for CQC inspection readiness and GMC Portfolio Pathway evidence, available now on Gumroad.

  • UK healthcare focus
  • Built by healthcare operators
  • GDPR-conscious by design
  • No adtech or tracking

01 — Who this is for

Calibrated for the desk every governance item lands on.

Portiko is built for the operational reality of UK healthcare — the practices and clinics where governance, evidence, and inspection readiness are run by named people with full diaries. The list below is who the tools are calibrated for, in settings and in roles.

In settings like

  • GP practices and PCNs
  • Care home groups
  • Private clinics and DSOs
  • Mental health services
  • Dental practices

Run by people who are

  • Clinical governance leads
  • Registered managers
  • Practice managers
  • Medical directors
  • Portfolio Pathway applicants

02 — What Portiko helps with

Three problems. Three Portiko tools.

Each card below names a concrete operational problem, the Portiko tool addressing it today, and the outcome a team using the tool gets. One alpha product, two live kits — and four named commitments at the foot of the page that hold across all three.

Problem

Clinical governance, scattered across spreadsheets

Risks, significant events, complaints, audits, governance meetings, and CQC-aligned evidence spread across folders, email, and shared drives — with nothing tying them back to the decision that produced them.

Tool · Closed alpha · pre-partner

Cadence

Fewer missed governance actions. Better meeting follow-through. Operational memory the next clinical governance lead inherits intact instead of having to reconstruct.

Problem

CQC inspection prep is a two-week scramble

Twenty-eight KPIs across the five CQC domains with no consistent tracking, no monthly review cadence, no inspector-ready packs. Inspection week becomes a sprint against scattered evidence.

Tool · Available now · £79

GovernReady

Reduced inspection scramble. A monthly governance cycle in place of a pre-inspection sprint. Evidence packs assembled in days, not weeks.

Problem

GMC Portfolio Pathway evidence over 18–24 months

Portfolio Pathway (formerly CESR / CEGPR) applications need eighteen to twenty-four months of structured evidence across the GMC's specialty framework. Most candidates assemble it ad-hoc, with no calibration on what 'good' looks like.

Tool · Available now · £79

CESR Evidence System

Faster evidence assembly. A submission-ready bundle instead of an 18-month folder maze. Calibration on what assessors actually look for, in time to act on it.

03 — The alpha product
Active alpha

Cadence — clinical governance, run as one system.

Risk register, significant events, complaint handling, audits, governance meetings, CQC-aligned evidence — captured once, triaged once, decided once, embedded once. Built for governance leads who want to lead, not chase.

Workflow

  1. 01

    Capture

Significant events, complaints, audits, and incidents land in a single intake. One form, one queue, one source of truth.

  2. 02

    Triage

Each item is categorised, scored, and routed to the right register or owner. Cadence carries the schema; the lead carries the judgement.

  3. 03

    Decide

Governance meetings produce named decisions and dated actions, not minutes. Each decision references the items that drove it.

  4. 04

    Embed

Decisions update the risk register, policy library, and SOPs. The next meeting opens with last meeting's actions, not a blank page.

Eight modules

  • Incidents
  • Complaints
  • Audits
  • Risks
  • Actions
  • Meetings
  • Evidence library
  • Policies

04 — What Cadence helps produce

Four real surfaces from the live build.

Captured against the Meadowview demo seed in the running Cadence app, against the same code partners sign in to. Every artefact below has structural linkage back to the record that produced it — no free-text claims, no fake data.

Cadence incident detail for INC-2026-008 — a repeat abnormal HbA1c result not followed up at 14 days. Clinical-care theme, high severity, patient-safety flagged, root-cause analysis under way.
Significant event with theme, severity, and audit trail.
Cadence governance meeting for MTG-2026-04-14 — four decisions, each linked to the incidents, risks, or policies that drove them, with rationale captured per decision so future readers can re-litigate or honour them without context loss.
Governance meeting with decisions linked back to source records.
Cadence policy detail for POL-CL-12 — abnormal results handling SOP, status under-revision, review 389 days overdue (red), framework attribution CQC + CQC-SAF + SAFE + EFFECTIVE.
Policy with currency tracking and CQC framework attribution.
Cadence stewardship surface — workspace stewardship register naming who has governance access, with a flagged warning that the workspace is currently single-stewarded (a single absence would block access changes, recoveries, and policy approvals).
Stewardship register — multi-steward redundancy as institutional posture.

05 — The Portiko library

Four additional systems alongside Cadence.

Cadence is the alpha product. Relay, Quorum, Beacon, and Atlas extend the library: daily clinical workflow, committee governance continuity, mechanically-computed assurance posture, and cross-system architectural integrity. Each is preview status — engineering-verified, not yet partner-deployed.

Relay

Preview

Daily clinical workflow

The daily operational app — sister to Cadence's governance side.

Pathology results, referrals, safety-nets, medication changes, recalls, and tasks land in a single inbox. Each capture flow writes a transactional audit row so nothing happens without a trace. Built for the desk where the day-to-day operational queues come together.

Six capture flows · SQLite + Drizzle · transactional audit log.

Quorum

Committee governance continuity

Governance obligation continuity for institutions with named committees.

Schemes of delegation, named decisions, minutes, obligation calendars, and authority-scope enforcement — built so the next chair inherits institutional context, not a folder of PDFs. Tenancy is institutional-identity (Person, Role, Committee, Membership), not flat workspaces.

Append-only chronology · 482 acceptance tests · institutional-identity tenancy.

Beacon

Assurance posture

Inspection-bearing evidence computed from real operational state.

Beacon reads Cadence, Relay, and Quorum's operational data and computes assurance posture mechanically — no narrative claims, no retrospective evidence assembly. Postures get sealed into integrity-protected packs with sha-256 over canonical form; supersession checks tell you when an earlier pack has been overtaken.

Live evaluators for all three apps · sealed packs · supersession-aware.

Atlas

Preview

Cross-system integrity

Architectural invariants verified in CI across the Portiko library.

Tenancy posture, schema discipline, cross-app reference resolution, polymorphic dispatch — Atlas declares what every Portiko app must be true about, then fails CI closed when any app drifts. The library composes as one library rather than four parallel codebases.

5 invariants across 4 apps · polymorphic reference dispatch · 13 acceptance tests.

06 — Available now

Standalone kits you can buy today.

The CESR Evidence System and GovernReady are working kits, available on Gumroad as one-off purchases. They cover the two healthcare-operations problems above where the right deliverable is a structured document set rather than an operating system — and the revenue funds the closed-alpha work on Cadence.

07 — Working with Portiko

Private alpha. Selected partners.

Cadence is currently being developed with a small number of named UK GP and clinic teams. Alpha partners get founder-level support, free access throughout the alpha, and meaningful influence over the general-availability roadmap. The alpha framing is deliberate: we'd rather build the system right with the first three practices than be open to the next three hundred before the operational truth lands.
